Wellness Recovery (WR) is a family-owned and operated, licensed inpatient substance use treatment facility offering care in a small, intimate 19-bed setting. We serve adults aged 18 and older, providing short-term Withdrawal Management (Detox) and Residential Rehabilitation services in a compassionate and structured environment. As a close-knit team, we pride ourselves on delivering truly personalized care. With 24/7 medical support, clinical counseling, and therapeutic programming, we help individuals not only stabilize physically but also begin the deeper emotional and psychological healing process.

Our mission is to improve community health and well-being by providing accessible, high-quality treatment for adults facing substance use disorders. Guided by a person-centered approach, we address the full spectrum of recovery, including the physical, emotional, and psychological, through integrated medical care, evidence-based therapies, and supportive services.

Wellness Recovery (WR) concluded its sixth full calendar year of operation in 2025, continuing its
mission to provide evidence-based care to individuals with substance use disorders. Grounded in the
understanding of addiction as a brain-based condition requiring both medical and therapeutic
intervention, WR remains committed to increasing access to care for underserved and non-dominant
populations. Interdisciplinary staff, including counselors, nurses, care managers, certified recovery
specialists, and medical providers, delivered 24/7 services in alignment with standards set by the
American Society of Addiction Medicine (ASAM). In partnership with Tracy Counseling Center, WR
continued to offer no-cost inpatient and outpatient services to first responders, recognizing the elevated
risk associated with chronic occupational stress and trauma exposure.
Throughout 2025, WR continued to build upon the operational and clinical advancements established in
the prior year. Under the leadership of Program Director and COO Tyler Tracy and Clinical Director
Alexandra Leopold, the program maintained updated policies, procedures, and treatment protocols
aligned with current regulatory expectations and emerging best practices. Efforts remained focused on
improving care quality, strengthening staff training, and enhancing coordination across systems. WR
also sustained and expanded key community partnerships, including ongoing collaboration with Single
County Authorities (SCAs), to support access to services and continuity of care. In addition, WR
launched a new website, implemented an electronic intake system, and introduced an online application
process, significantly improving accessibility to treatment by streamlining referrals, reducing barriers to
entry, and increasing efficiency in the admissions process.
Clinical programming remained rooted in an integrated model combining medical management with
individualized therapeutic intervention. Under the direction of Medical Director and Psychiatrist Dr.
Sudhir Rao, WR’s medical team provided psychiatric care, withdrawal management, and support for co-
occurring conditions. Treatment services included individual and group therapy, peer support, life skills
development, and psychoeducation. Emphasis was also placed on discharge planning and continuity of
care through coordinated transitions to outpatient providers and community-based supports.
WR staff continued to engage in ongoing professional development to support high-quality service
delivery. Core trainings included CPR/First Aid, Naloxone administration, confidentiality, infectious
disease control, motivational interviewing, crisis intervention, and harm reduction. Additional training
areas included medication-assisted treatment (MAT), trauma-informed care, co-occurring disorders,
Adverse Childhood Experiences (ACEs), health disparities in SUD treatment, and culturally responsive
practices. Continued investment in Certified Recovery Specialist (CRS) training further strengthened
WR’s peer support model.
In 2025, WR served a comparable number of individuals relative to the prior year, with a slight, non-
significant decrease in total admissions. Notably, outcome indicators demonstrated meaningful
improvement, including a significant increase in program completion rates as well as a longer average
length of stay and total days in treatment. These trends reflect increased patient engagement, retention,
and stability throughout the course of care. Patients served ranged broadly in age and background,
reflecting the diverse needs of the surrounding communities. Additional demographic and clinical data
are presented in the following section of this report. WR remains committed to providing accessible,
high-quality care and to continuing to strengthen treatment outcomes in the year ahead.

At Wellness Recovery (WR), we are dedicated to providing comprehensive, person-centered treatment
for individuals aged 18 and older experiencing substance use disorders. Our integrated approach
combines evidence-based medical treatment, psychotherapy, peer support, and wellness-based therapies
to foster long-term recovery and overall well-being. We believe that recovery is more than just
abstinence—it’s about rebuilding lives through education, empowerment, and healing. Our
multidisciplinary team develops individualized treatment plans that address the medical, psychological,
and social aspects of addiction. Patients engage in individual and group therapy, psychoeducational
classes, life skills training, and structured support systems, covering key topics such as nutrition, stress
management, relapse prevention, and goal setting.

II. Our Treatment Approach
At WR, we provide a comprehensive and individualized approach to addiction treatment, integrating
evidence-based practices with medical care. Our services include individual counseling, which offers a
safe space to process trauma and build a foundation for recovery, and group counseling, where
participants can connect with peers, develop social coping skills, and explore the impact of addiction on
their lives. Family integration sessions focus on strengthening support systems and creating
collaborative discharge plans while our medical treatment team addresses withdrawal symptoms and
overall mental health needs. In addition, mental health counseling ensures that participants explore the
critical connection between mental health and substance use disorders. WR also offers recreational
therapy to promote physical, social, and emotional well-being through activities like art, music, and
movement therapy. Certified recovery and peer support specialists provide guidance and encouragement
based on their personal recovery journeys, fostering hope and resilience. Our case management services
help patients navigate treatment and connect with community resources to support their transition back
into everyday life. Complementing these services are diverse group offerings, such as morning
meditation, psychoeducation, health education, and life skills training, which empower participants to
build healthy habits and sustain long-term recovery.

sexual orientation, spiritual beliefs, or socioeconomic background. At WR, we believe that

IV. Our Integrated Service Model
At WR, we utilize a comprehensive Integrated Service Model designed to address the diverse and
complex needs of individuals in recovery. Our approach seamlessly combines medical care, evidence-
based counseling, wellness-based therapies, and long-term support, ensuring that each patient receives
personalized, continuous care throughout their recovery journey. We offer two core programs:
Residential Rehabilitation and Withdrawal Management (Detox Services). Through a structured yet
flexible framework, our integrated model meets individuals where they are in their recovery process,
providing the medical, psychological, and social support needed for lasting success.
Residential Rehabilitation Program: Our residential program offers structured, 24/7 care in a supportive
environment where individuals can focus on recovery free from outside distractions. The following
services, among others, are offered:
● Individual Counseling – One-on-one sessions tailored to address trauma, substance use, and co-
occurring mental health challenges.
● Group Counseling – A core component of treatment, where individuals gain insight, receive peer
support, and develop social coping skills.
● Family Integration – Strengthening family support systems and preparing for long-term recovery
through structured family sessions.
● Medical & Mental Health Treatment – On-site medical and psychiatric care, including
medication management and wellness education.
● Recreational Therapy – Engaging activities like art, music, yoga, and movement-based therapy
to enhance emotional regulation.
● Case Management & Peer Support – Guidance from certified recovery specialists, assistance
with securing housing, employment, and other critical resources.
● Life Skills & Psychoeducation – Training in stress management, goal setting, health education,
and financial literacy to support long-term independence.
Withdrawal Management & Short-Term Detoxification: For individuals requiring medical supervision
during the withdrawal process, our detox program ensures a safe and supportive transition into
treatment. Typical services include:
● 24/7 Medical Supervision – Around-the-clock monitoring by a medical team to ensure safety and
comfort during withdrawal.
● Medication-Assisted Support – Where appropriate, medications are provided to ease withdrawal
symptoms and support stabilization.
● Individual & Group Counseling – Psychoeducation and emotional support to prepare individuals
for continued treatment.
● Wellness-Based Therapies – Mindfulness, relaxation techniques, and physical movement to
promote overall well-being.
WR’s Integrated Service Model ensures that every individual receives comprehensive, person-centered
care designed to facilitate long-term recovery and sustained wellness. Our approach recognizes the
biopsychosocial nature of addiction, addressing medical, psychological, social, and emotional needs
through a multidisciplinary treatment framework.
V. Patient Satisfaction
Patient satisfaction remained a cornerstone of our operations in 2025. Feedback was collected through
anonymous online and physical questionnaires with general satisfaction rated high, specifically
highlighting individualized care and extra effort demonstrated by staff, reinforcing WR’s core values of
empathy and non-judgmental care.
Demographic Report
In 2025, a total of 298 unique individuals were served, reflecting a slight decrease from the previous
year. Among these, 276 individuals had a single admission, while 22 individuals had multiple admission
stays, contributing to a total of 52 admissions for this group. This brings the total number of admissions
for the year to 328, including all single and multiple admission stays.
● Biological Sex: 64% male (191 individuals), 36% female (107 individuals).
● Age: The average overall age was 41 years, with males averaging 42 years and females 39 years.
The age range of patients spanned from 19 to 73 years.
● Race/Ethnicity: The majority of individuals identified as White (80%), followed by
Black/African American (12%), Multiracial (4%), Hispanic (2%), and Other (2%).
● County of Residence: Most individuals were from Fayette County (54%), followed by
Allegheny (13%), Washington (12%), Westmoreland (12%), and Greene (4%).
A total of 502 substance use disorders (SUDs) were treated in 2025, continuing to reflect a high
prevalence of polysubstance use:
● 58% of individuals (173) were diagnosed with a single substance use disorder.
● 30% (89 individuals) had two substances involved.
● 12% (36 individuals) had three or more substances.
The most commonly treated substance use disorders included Stimulants (36%), Opioids (29%), Alcohol
(18%), Amphetamines (11%), and Sedative/Hypnotic Use (6%).
WR provided a total of 5,436 admission days across all patients, reflecting a notable increase in overall
duration of care. The average length of stay increased to 18 days, with a median stay of 12 days.
Additionally, 168 individuals successfully completed the program, representing a significant increase in
successful program completions and improved treatment engagement and retention. The majority of
services were funded through Carelon (66%), followed by CCBH (20%), FCDA (7%), WCDA (1%),
and Other (6%).
Community Outreach
Outreach activities continued across Fayette, Westmoreland, Washington, and Greene Counties. WR
staff participated in the annual Community Block Party for Recovery held in July on Main Street in
Uniontown, maintaining a visible presence in local recovery-focused initiatives. WR also supported
workforce development by hosting graduate-level trainees for clinical field experience, including
students from PennWest University and Duquesne University.
In support of care coordination, WR conducted outreach visits with hospital social work departments,
including Penn Highlands Healthcare and Uniontown Hospital, to review admission criteria, referral
processes, and continuity of care practices. WR also participated in community engagement events,
including National Night Out activities across multiple counties.
WR continued its commitment to community investment through its annual $2,000 collegiate
scholarship for student-athletes demonstrating a commitment to a sober lifestyle at Kutztown University.
Additional contributions included financial support to a local law enforcement agency’s K-9 program
and sponsorship of a youth athletic team. Facility leadership remained actively involved in regional
initiatives, with CEO Tyler Tracy continuing participation in the Westmoreland County Opioid
Coalition.

Important Notations and Future Prospects
In 2025, Wellness Recovery (WR) made measurable progress toward previously identified strategic
priorities, with a focus on expanding access, improving care coordination, and strengthening community
engagement. WR successfully launched a new website and implemented an electronic intake and online
application system, significantly improving accessibility to services and streamlining the admissions and
referral process for both patients and providers.
WR also continued to expand its presence and capacity through strengthened partnerships and ongoing
efforts to increase contractual relationships with Single County Authorities (SCAs), particularly in
Westmoreland and Armstrong counties. These efforts have supported broader regional access to
treatment and improved coordination of care across systems. In parallel, WR increased investment in
workforce development, including expanded opportunities for staff to obtain Certified Recovery
Specialist (CRS) credentials, further integrating peer support into the treatment model.
Community engagement efforts were also expanded, with increased participation in outreach events and
collaboration with local organizations to enhance visibility, reduce stigma, and connect individuals to
care. WR has also begun to establish a more consistent digital presence to support outreach and
education efforts.
Looking ahead, WR is actively exploring opportunities for programmatic and physical expansion to
meet growing community needs. This includes evaluating the feasibility of expanding facility capacity
as well as developing recovery housing options to support individuals transitioning from inpatient
treatment to more independent living environments. These initiatives are intended to improve continuity
of care, promote long-term stability, and reduce relapse risk.
WR remains committed to thoughtful, sustainable growth, with continued emphasis on improving
treatment outcomes, increasing accessibility, and responding to the evolving needs of the communities it
serves.
